The initial consultation is an opportunity for you to have the time to discuss all your health concerns and health goals, and to have a wider discussion about you and your life.
. During the initial consultations there will be an extensive health and wellbeing history and a lifestyle based physical examination. Further health checks as well as individualised tests for health markers and nutritional status, would be discussed with you if needed. Based on this we will create health targets and formulate a realistic and a sustainable plan to achieve these over subsequent consultations as frequently as you wish.
My main aim is that we do this together, as only a plan you are fully happy with and invested in will work. I will make sure I develop a customised plan that works for you and your lifestyle.
The consultations are aimed to instil within you the skills and motivation to move forward with a renewed energy to achieve your chosen health challenges together.
It is important that you are supported throughout the process until your long term goals are achieved. There is no one size fits all plan, so we would meet regularly to monitor your progress and adjust when needed.
Planning for and predicting potential barriers to change will help minimise relapses. My role is to support and coach you through, using motivational counselling and setting realistic targets. Those targets will also be measurable, so you can see and feel real change as you continue on your health journey.
Time limited health programs are popular but often relapse occurs once the program has ended. Having a Lifestyle Medicine Practitioner is different. To achieve a sustained change we need to be maintaining new health habits for at least 6 months. Each individual is different however and ongoing health coaching will continue for as long as needed for maintenance to be achieved.
